Urokinase
Indications/Uses
Thrombotic-occlusive diseases eg, cerebral thrombosis (w/in 5 days after the onset of symptoms & w/o hemorrhage detectable by computed tomography); peripheral arterial-venous occlusion (w/in 10 days after the onset of symptoms).
Dosage/Direction for Use
Cerebral thrombosis 60,000 IU once daily for 7 days. Peripheral arterial-venous occlusion 60,000-240,000 IU on 1st day of treatment followed by tapered doses for 7 days.
Contraindications
Intracranial & retroperitoneal hemorrhage, hemoptysis. Patients undergone intracranial or spinal surgery or w/ such injury (in the last 2 mth). Aneurysm; serious disturbances of consciousness; atrial fibrillation (particularly w/ mitral stenosis), infective endocarditis, old MI & patients w/ prosthetic valve; neurological signs & symptoms of stroke.
Special Precautions
Avoid in patients w/ a possibility of cerebral embolism or w/ a risk of bleeding. Monitor frequently hematologic exam eg, blood coagulability (bleeding time, prothrombin time). W/draw immediately if intracerebral hemorrhage is suspected. Patients w/ hemorrhage under surgical treatments eg, operations (including hepatic, renal biopsies), hemorrhagic oculopathy eg, diabetic hemorrhagic retinopathy, digestive tract & urinary tract hemorrhage, abortion of premature delivery; ulcers of the digestive tract, colitis, serious HTN, active TB, history of intracranial hemorrhage. Patients receiving anticoagulants; coagulation factor deficiency, thrombocytopenia. History of hypersensitivity to urokinase or tissue culture urokinase. Serious hepatic or renal disturbances. Pregnancy. Childn. Eldelry.
Adverse Reactions
Bleeding, shock, hypersensitivity reactions eg, rash & urticaria; elevated levels of SGOT or SGPT; nausea, vomiting, anorexia; fever, chill, headache, malaise.
Drug Interactions
May intensify bleeding tendency w/ anticoagulants (eg, warfarin), drugs w/ platelet anti-aggregant effects (eg, aspirin, dipyridamole, ticlopidine HCl).
